PHOENIX — Authorities say a man accused of driving impaired is facing two counts of second-degree murder after a fatal head-on crash in Phoenix.

Maricopa County prosecutors say 28-year-old Octavio Grado-Campos also was arrested Tuesday on suspicion of two counts of aggravated assault.

He didn't have a lawyer at his initial court appearance Wednesday when his bond was set at $750,000.

Phoenix police say the vehicle Grado-Campos was driving Sunday night collided with a car occupied by four people.

Two of those people — a 28-year-old woman and 27-year-old man — died from their injuries.

Authorities say a 4-year-old girl sustained serious spinal injuries while a 26-year-old man suffered multiple fractures.

Police say lab results show Grado-Campos' blood-alcohol level was at 0.241 percent. The legal limit for Arizona drivers is 0.08 percent.
