Phyllis Ann Cook
Phyllis Ann Cook, 84, of Bargersville, passed away Wednesday, Nov. 27, 2013.
The daughter of Cornelius Sr. and Elizabeth A. Kuehn O’Hern, Phyllis was born Feb. 4, 1929, in Linton.
A homemaker, she was a member of Mount Pleasant Christian Church.
Survivors include a son, Brian (Debby) Cook of New Palestine; a sister, Marilyn Rose (Jim) Summers of Indianapolis; two grandchildren, Kayla Cook and Sean Veteto; a great-grandson, Trevor Veteto; a niece, Angela (Jake) Pope; two nephews, David A. (Sally) Miller and Ronald Miller; and several other nieces and nephews.
Besides her parents, Phyllis was preceded in death by her husband, Gleddis “Pete” Cook; two brothers, Bobby O’Hern and Cornelius O’Hern Jr.; and three sisters, M. Jane Miller, Janet Lee Allen and Betty O’Hern.
Services will be at 1 p.m. Tuesday at Forest Lawn Funeral Home, 1977 S. State Road 135, Greenwood. Friends may call from 11 a.m. until service time Tuesday at the funeral home. Entombment will be at Forest Lawn Memory Gardens in Greenwood.
Memorial contributions may be made to the American Heart Association, in care of the funeral home.
