INDIANAPOLIS
Rose Ann Phelps, 65, of Indianapolis, died Sunday, Sept. 4, 2016, at Franciscan St. Francis Hospital in Indianapolis.
Survivors include her husband, Ken Phelps; sons, Brian (Jennifer) Phelps, Daniel (Stacey) Phelps; three grandchildren and three siblings, Peggy (David) Smith, Joe Page and David (Amanda) Page.
A funeral service will be at 1 p.m. Friday at Calvary Baptist Church, 200 Sunset Blvd, Greenwood. Visitation will be from 11 a.m. until service time Friday at the church. Entombment will follow at Forest Lawn Memory Gardens.
Memorial contributions may be made to Calvary Baptist Church.
Singleton Community Mortuary and Memorial Center is handling the arrangements.
