INDIANAPOLIS
Jason Dewayne Simmons, 38, of Indianapolis, died Sunday, Aug. 21, 2016.
Survivors include his wife, Patricia (Sizemore) Simmons; children, Alicia Sizemore, Rebecca Young and Jacob Smith; stepparents, Ronald and Roberta Stinson; sisters and brothers; and four grandchildren.
Jason was preceded in death by his stepson, Cody Sizemore.
Visitation will be 11 a.m. to 1 p.m. Thursday, with the funeral service starting at 1 p.m. at G.H. Herrmann Madison Avenue Funeral Home, 5141 Madison Ave., Indianapolis.
Memorial contributions may be made to Riley Hospital and/or the Ronald McDonald House.
