INDIANAPOLIS
Hazel Wren Lewis, an infant, of Franklin, died Thursday, Aug. 4, 2016, in Indianapolis.
Survivors include her parents, Mike and Sarah (Collins) Lewis of Franklin; sister, Violet Lewis and brother, Oliver Lewis both of Franklin; grandparents, Michelle and Gene Collins of Martinsville and Michael Lewis of Columbus and Joy Lewis of Florida.
A celebration of life service will be conducted at 6:30 p.m. Thursday at Cornestone Church, 1000 W. County Road 375 South, Trafalgar.
Jessen Funeral Home, Franklin Chapel, is entrusted with arrangements.
