Jerry Smith, 61, of Indianapolis, died Sunday, March 13, 2016.
Survivors include his mother, Mary L. (Reynolds) Smith; and brothers, Thomas M. (Patricia) Smith, Robert (Janet) Smith, Denny (Susie) Smith and Rick (Debra) Smith.
Visitation will be 11 a.m. to 1 p.m. Thursday at G. H. Herrmann Madison Avenue Funeral Home, 5141 Madison Ave., Indianapolis, with services following at 1 p.m. at the funeral home.
Memorial contributions may be made to the American Lung Association or Humane Society.
