Tiffany Marie Madsen Smith, 33, of Indianapolis, died Saturday, July 11, 2015.
Survivors include her children, Bailey Michael Gray, Blake Anthony King and Olivia Victoria Dunham; parents, Charlie and Kimberly Madsen; sisters, Tabitha S. and Tara L. Madsen; and grandmother, Nora Madsen.
A service will be at 11 a.m. Thursday at Singleton Community Mortuary and Memorial Center. Visitation will be from 4 to 8 p.m. Wednesday at the mortuary. Interment will be in Greenwood Cemetery.
