INDIANAPOLIS
Hazel B. Adkins, 87, of Indianapolis, died Tuesday, Jan. 13, 2015.
Survivors include two children, Charlene (Steve) Hendon and Jerry (Kelly) Adkins; five grandchildren; one great-grandson; and a brother, Arvis Baugh.
She was preceded in death by her husband, Leonard Lee Adkins.
The service will be conducted at 11:30 a.m. Saturday at G.H. Herrmann Madison Avenue Funeral Home, 5141 Madison Ave., Indianapolis. Visitation will be from 5 to 8 p.m. Friday at the funeral home. Burial will be at Forest Lawn Memory Gardens in Greenwood.
Memorial contributions may be made to the Indiana Parkinson Foundation, in care of the funeral home.
